What is sciatica?
Sciatica is the name for pain caused by irritation or compression of the sciatic nerve, the longest nerve in the body, which runs from the lower back down through the buttock and the back of each leg. It usually shows up on one side and can range from a dull ache to a sharp, electric, burning sensation. Most people find it gets worse on sitting, bending, sneezing or staying still for too long. Numbness, tingling or weakness in the leg is also common.
True sciatica is a symptom rather than a diagnosis — the underlying cause matters. How quickly do most people improve?
Most patients with straightforward mechanical sciatica notice meaningful change within two to four sessions. Acute, severe sciatica may need a few more sessions and a slightly longer recovery curve. Long-standing or recurring sciatica usually responds well too — it just takes more work to undo the patterns that keep bringing it back.
You will never be recommended more sessions than you need. If something isn't responding the way it should, we'll be honest with you about it and refer onward to an MRI or GP review where appropriate.
When to seek help urgently
Most sciatica resolves with conservative care like osteopathy. There are a few signs, however, that mean you should call 111 or go to A&E rather than wait for an appointment:
- Loss of bladder or bowel control
- Numbness around the saddle area (groin / inner thighs)
- Severe weakness in both legs
- Sudden severe pain after a fall or accident
- Fever alongside back pain
